protect your health, you need to take care of yourself - and
put you first. The YouFirst™ Health Risk Assessment (HRA)
is an internet-based survey that provides a measure of relationship
between behaviors and outcome related to health status. The
survey asks about family history, general health parameters
(weight, blood pressure, cholesterol levels, etc.) and lifestyle
behaviors (diet, tobacco and alcohol use, safety precautions,
etc.). The risk assessment allows you to surface risks you may
have so you may take the appropriate action to manage and reduce
your health-related risks.
Your answers to the survey will be used to prepare a personal
health report that is returned to you. Your report will tell
you where you stand, identify health conditions or habits that
may lead to health problems later and lists steps you can take
now to improve your health. All information that you provide
and your personal health report are strictly confidential. However,
if you chose, you can share your HRA with a FutureHealth®
care management nurse so she can work with you to help you reduce
and manage your health risks.
